Community Support Worker - Assertive Outreach
Bridgwater
Who are we?
Livewell Southwest is an independent, award-winning social enterprise providing integrated health and social care services for people across Plymouth, South Hams and West Devon, as well as some specialist services for people living in parts of Devon and Cornwall.
We are at the forefront of integrating health and social care, which means that we care for people in new ways that are more efficient, with health and social care professionals who would have previously worked in individual teams now working together.
Livewell Southwest is an employee-led organisation, and the largest community health and social care enterprise in the UK. We employ more than 3,000 members of staff who live and work in the local area and champion their progression, development and commitment to providing outstanding care.
At Livewell we are encourage potential applicants to contact the team for an informal chat about the role, before completing a formal application.
Where are we?
Our award-winning services are diverse, including community nursing, services for people with learning disabilities, physiotherapy, community mental health services, social work, wellbeing and health improvement, services for children and young people, and community pharmacies. We’re able to provide these services both within people’s homes and from one of our 12 locations based across the south west.
At Lee Mill on the outskirts of Dartmoor, near
Ivybridge, we have a 12-bed, low-secure mental health unit. And Plym Bridge
House in north Plymouth is our 12-bed, purpose-built psychiatric unit for
teenagers with severe mental health problems or mental illness. We’re fortunate
to have inpatient facilities available to support our communities.
