What I find most rewarding about my role is seeing the difference that we can make to individuals.
Our Programme Director, Anne, is passionate about the difference our work within our SWPC can make to people’s lives. In this video, you can hear how one of our patients was able to reconnect with their family, their care teams, and their community after we made local services available which met their specific needs.
