Administrator- North & East Integrated Transfer Of Care Hub (iToCH)

Job summary

We have a very rare opportunity to join our North and East Integrated Transfer of care Hub (iToCH) as one of our administrators to support the hub with processing all the referrals coming into the North and East community service. This role will be based in Carew House to work alongside the neighbourhood iToCH.

The Administrator will be responsible for reviewing key blockages to patient flow, highlighting key issues to Team Managers maintainingaccurate and timely feedback to referrers, liaising with multi-agencies for the co-ordination of patient flow.You must possess excellent communication and people skills; have the ability to work under pressure, whilst being a committed member of ourlocal point of access team.

This is a full time position covering the hours of 8am to 8pm 7 days/week, including bank holiday along with the other administrator within the team.

Main duties of the job

The role of the Admin Support Worker is to work with prioritisers who are Nurses, Social Workers, Therapists and other members of staff employed within the multi-professional team, processing and managing the flow of patient referrals through the Transfer of care hub and local point of access.

The Admin Support Worker will monitor the referrals on the single electronic referral system on a continual basis, triaging in partnership with the multi professional team.

  • Good standard of education including GCSE English & Maths or equivalent

Experiece, skills and aptitude

  • Previous experience in a healthcare office environment
  • Computer literate with a excellent working knowledge of Microsoft Office, Excel and Powerpoint
