Support / Peer Support Worker

Job summary

To work in a award winning team to provide early intervention, prevention, outreach nursing care and support for people who are experiencing homeless or rough sleeping across the whole of Somerset. This includes those in all supportive/temporary/unsecure accommodation.Key duties include active engagement and maintenance of relationships; develop and promote positive joint working practice with other services including the community mental health team and drug and alcohol services, police, probation, council etc... You will be expected to take patients to their appointments in your own vehicle and help them re engage with health/social services.You will work as part of a team making every contact count, taking every opportunity to impact directly and indirectly on the health and wellbeing of people. You will be able to work under direction and as an autonomous practitioner as agreed with the clinical lead you will demonstrate safe decision-making and compassionate, non-judgemental support.

  • Must be able to communicate in English Language, both written and verbally appropriate to the post.

Desirable

  • Basic Life Support.
  • Level 2 English and Maths.
  • (equivalent to GCSE grade 4 or above).

Experience

  • Experience of dealing with the public and other organisations.
  • Understanding of national issues that will influence care and practice with people who are homeless and rough sleeping.

Desirable

  • Knowledge of one or more of the following: Homelessness, Housing Law, safeguarding, mental health, sexual health, substance misuse and the criminal
  • justice system.

  • Full driving licence and use of a vehicle, and able to drive and travel across the county as required utilising own transport and claiming accordingly.
