Are you an enthusiastic and flexible dietitian?
We are looking for a dietitian specialising in diabetes to join our progressive community team.
You'll be managing your own caseload of community patients, working mostly in clinic outpatient, but you'll also have some home visits and remote working thrown in also!
This is an exceptional opportunity to gain broad clinical experience both within the specialist diabetes service and the wider community dietetic team, providing an excellent foundation to develop your future professional career.
A driving licence and access to your own vehicle is essential for this role.
Main duties of the job
You will provide a dietetic service to patients referred to the Diabetes Intermediate Care Team. Service delivery will be via outpatient clinics, Structured Group Education as well as domicilliary visits as required. Providing training other health care providers may also be required according to demand and identified need.
You will also have a caseload of patients with a range of long-term conditions referred to the Service by GP's, Consultants, Community Nurses, AHP's and community hospitals as part of the general community clinic service. Domiciliary visits may also be required.

Qualifications
Essential criteria- BSc Nutrition and Dietetics or BSc Degree and Post Graduate Diploma in Dietetics
- HCPC registered
- Post Registration training, Diploma of Advanced Dietetic Practice Accredited or equivalent
Experience
Essential criteria- Experience of dietetic service delivery in a variety of different settings, including community work
- Experience of Group Education
- Evidence of behaviour change or motivational interviewing skills
