Primary Care Network Occupational Therapist

Job summary

Come to Somerset and be part of an exciting opportunity to contribute to improving population health, through innovation as an Occupational Therapist in Primary Care! This role in Bridgwater Bay Primary Care network already has an Occupational Therapist, so the PCNs understand how they wish to expand the current provision for their local populations and the value OT can bring to the team. As stated by the PCN themselves:- 'We are looking for a second OT to support our two well-established PCN workstreams, Enhanced Health in Care Homes and Proactive Care in the Community. You will work alongside a frailty nurse, complex care GPs, pharmacists and care co-ordinators. We operate an MDT for each workstream with community patients often identified using Brave AI to help us proactively find those patients who are not generally known to their GP. The Care Home work is focused more generally around providing advice, information and support to care home staff on a variety of subjects. The postholder will need to work well as part of a team, be flexible in their approach and ideally would have frailty experience and understand working with a population who experience deprivation and inequality,'

As a host model, Primary Care provide daily operational support to delivery and Somerset NHS Foundation Trust support with supervision, professional support and career development.
