Job summary
Are you a former Adult/General nurse missing the challenges and rewards of nursing? Would you like to return to practice and re-join the NMC professional register?
If the answer is yes, we are currently recruiting former registered nurses who are looking to return to practice and regain their professional registration within District Nursing.
Within this exciting post, you will be supported to complete a Return to Practice programme at Plymouth University. The course will be funded, and you will be employed by Livewell Southwest as a Band 3 Healthcare Assistant £21,730 per annum, pro-rata for the duration of your training. It is recommended that due to the nature of the programme, employed hours are a minimum of 30 hours per week.
On the successful completion of the course, and receipt of NMC PIN number, you will work as a band 5 registered nurse within your allocated team.
You will have study leave to attend university as required, and you will be supported in practice to achieve your practice learning outcomes. During the course, you will be expected to complete a minimum of 450 clinical hours within your allocated team and commit to attending 8 study days. Main duties of the job
We have Return to Practice placements available in our District Nursing Service and Plym Neuro Rehabilitation Unit . Please indicate a preference with regards to what team you would like to be based within the qualification section of your application.

Responsible for assessing, planning, evaluating and implementing care which may be complex, sensitive, unpredictable with the support of a Registered Practitioner.
To undertake and to develop all nursing skills and competencies, updating as required for their role to enhance the skill mix of the nursing team ensuring that the ever-changing health needs of patients are met.
Returnee will be designated to take charge under supervision (if appropriate to work area)
The Returnee is required to be able to teach unqualified staff, including pre and / or post registration students.
The returnee will always work within the guidelines of the NMC code for trained nurses and organisational policies.
All Livewell Southwest staff are expected to able and willing to work across a 7 day service.

Key Tasks and Responsibilities of the Post:
- Duties all duties are under the direct supervision of a registered nurse/practitioner
- Provide holistic evidence based / best practice care to patients in accordance
- with National and Organisational approved policies/procedures and individual care plans.
- Utilise nursing skills to assess, plan and implement and deliver care to patients and review care to patients appropriately.
- Ensure planned nursing care is implemented and evaluated; take appropriate action in the light of evaluation.
- Undertake comprehensive risk assessments of all situations associated with the care of patients in order to ensure nurses and carers safety. This may include assessment and provision of equipment ensuring it is used safely.
- Monitor and maintain the health, safety and security of self and others.
- Play an active role in multi-disciplinary reviews, reporting on the nursing assessment of the patients progress and participating further in Multi-disciplinary Care Planning.
- Be involved in referrals, discharge planning, and case conferences to support or manage appropriate care provision for patients.
- Maintain accurate records, which are confidential, up to date, legible and that all care given must be documented. These records may be paper or IT based system and must be maintained as specified in the Organisation Policies, NMC guidelines and Government directives.
- To function as a member of a multidisciplinary team taking appropriate responsibility for the implementation of decisions made by the team.
- Provide education and continuing support to patients, family and carers.
- Undertake health promotion as requested to all age groups and contribute to planned health promotion activities.
- Be aware of organisational, professional and national developments within primary care and work within these guidelines e.g., NSF/ NICE/ NMC.
- Be aware of and abide by organisations approved Policies, Patient Group Directives, Standards and Quality Assurance Initiatives.
- The returnee is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations/ Chief executive office).
- Be conversant with the vulnerable adult and child protection procedures and policies of Livewell Southwest.
- Always maintain confidentiality in respect to the patient.
- Be responsible for organising own time management daily in line with caseload demands balancing patient needs with the administrative aspect of the role.
- Initiate and participate in Clinical Supervision.
- To liaise effectively with other areas and services. To develop and maintain relationships with providers statutory and non-statutory.
Share knowledge and information to promote a cohesive team.
- Act as a professional role model demonstrating clinical skills and high standards of practice and professional conduct.
