Job summary
22.5 hours per week. We are seeking to recruit a Community Support Worker to work as part of the new multi-disciplinary Core Community Mental Health Service delivering specialist mental health interventions to both individuals/groups

You will be joining an enthusiastic Multi-Disciplinary Team which include Doctors/Nurses/Social Workers/Psychologists/Occupational Therapists/Peer Support Workers/Admin Staff, who provide regular supervision & reflective practice
You will have the opportunity to work autonomously whilst under the direction of a Qualified practitioner to provide recovery orientated evidenced based interventions to people with severe & enduring mental health difficulties & their carers, enabling them to access mental health specialist support when required in line with Recovery & Trauma Informed Principles
You will be joining the Community Mental Health Services at an exciting time of change influencing how Community Mental Health Care will be delivered & leading local services in providing outstanding care in the community for Service Users
Main duties of the job
You will work closely with other statutory and non statutory agencies to provide specialist mental health assessment and evidenced based treatments (both individual and group work based) for people with severe and enduring mental health difficulties and complex presentations through. You will deliver care in line with agreed principles and policies, (Personalised Care Framework)
The post holder will co design plans of care with service users, their carers and the multi-disciplinary team,will be responsible for maintaining accurate clinical records, participating in mandatory training and line management and caseload supervision, will contribute to the maintenance of a positive learning experience for students and a psychologically safe environment for team members.
The post holder will manage their caseload independently, being responsible for managing their own electronic diary, and scheduling appointments to meet the demand of the job. You will be able to travel across the city and undertake home visits to service users during the core working hours.
This role may not be eligible for sponsorship under the Skilled Worker route.

Key Tasks and Responsibilities of the Post
You will work as part of a Multi Disciplinary Team in accordance with Livewell policies in order to work, enabling them to access mental health specialist support when required in line with Recovery and Trauma Informed Principles.
You will be involved in the co-production of holistic, evidenced based, care plans in line with the Personalised Care Framework.
You will, under the guidance of a registered professional, provide holistic evidenced based recovery focused and trauma informed care to individuals and groups, and support other agencies to contribute to this care, maintaining accurate clinical records.
You will provide interventions to a variety of people and their families known to the CMHT enhanced team, to promote recovery and resilience.
You will maintain links with people admitted to hospital in order to they maintain links to their community where possible.
You will attend and contribute to multi-agency forums in relation to service provision and individual care of service users.
You will practice within the legal and ethical framework set out by mental health related legislation to ensure that service users interests and well-being are at the fore.
You will work collaboratively with other statutory and VCSE organisations in line with relevant policies, you will liaise, advise, develop and maintain good relationships with service users, carers, relatives and fellow health, social care professionals who are involved in the delivery of services.
On a day to day basis you will be supported by a shift lead who provides appropriate supervision/advice to others.
You will contribute to the maintenance of a psychologically safe and positive learning environment for all staff, providing clinical advice/ guidance and line management for junior staff/students as required and participating in training programs designed for all staff groups.
You will take individual accountability for your practice maintaining your own professional development, participating in appraisal, performance review and all activities aimed at keeping professional skills updated.
You will contribute to the monitoring of standards of practice within the team through the use of clinical reporting systems and participation in the services clinical audit programme, this will include annual Personalised Care Framework reviews, medication and Health Records audits.
You will contribute to the collation of evidence within the team in support of standards for better health improving quality.
You will actively support the team through change in organisation and service delivery through the implementation of the Personalised Care Framework.
You will actively encourage people and their carers to be involved in service development
You will seek information and advice from key people when outside of your area of expertise
Work collaboratively with the IPS providing information and support to enable service users to engage in meeting education, employment and vocational needs. To include arranging initial contact and attending with the service user as required

- Relevant experience of working with and supporting people with a primary diagnosis of severe and enduring mental health problems and their carers preferably in a community environment.
- Experience of delivering one to one and groupwork treatment orientated interventions.

- Minimum of 5 gcses grades a-c including maths and English
